Triniti Home Health & Hospice is jointly owned by Guardian Angels Senior Services of Elk River and Cassia, an Augustana/Elim affiliation. From the comforting care we provide in hospice, to the restorative focus we have in home health care, our synergistic teams set us apart from other agencies. Triniti Home Health & Hospice employs a community of health care professionals consisting of nurses, social workers, therapists, chaplains, music therapists, massage therapists, volunteers, and more. Whether you or your loved one is looking for care, our compassionate team is equipped to handle a variety of needs.

We serve people in Anoka, Carver, Dakota, Hennepin, Isanti, Mille Lacs, Ramsey, Scott, Sherburne, Washington, and Wright counties, as well as eastern Stearns, Benton, McLeod and Meeker counties in Minnesota. We are proud to have a continuously high recommended rate for both home health and hospice services according to a national survey.

Staying Safe Together

COVID-19 Protocols

At Triniti Home Health & Hospice, the health and safety of our patients, caregivers, and family members is our number one priority. For more than three decades, we've maintained rigorous, high-level safety protocols to help mitigate contagious illnesses and protect our community. 

What we're doing to keep you and your loved ones safe:

  • All caregivers wear face masks and eye protection for every visit when community transmission rates are high. Gloves and gowns are also worn as needed
  • All equipment is thoroughly cleaned and sanitized after each visit
  • Caregivers monitor their own temperature and symptoms and stay home if not well
  • Caregivers check the patient's temperature before the start of each visit.  We may ask that you wear a mask if you are able to tolerate it.  

Triniti Home Health & Hospice works hard to keep you and your loved one safe. Should there be a change in COVID-19 protocols, we will update our page with the latest safety requirements.
